Crossfield Registry is now a participating registry for the Alberta Health Care Insurance Plan (AHCIP).
You can bring your completed AHCIP forms and supporting documents to our office for processing.
We handle applications, updates (such as name, address, or marital status changes), and replacement card requests.
Health cards are mailed directly to you from the AHCIP office, and this is a free service. Forms are available at our office or can be downloaded and completed in advance.
How to apply
All new and returning eligible Alberta residents must be registered for the Alberta Health Care Insurance Plan (AHCIP) coverage to receive insured and physician services.
Step 1: Eligibility
To be eligible for AHCIP you must be legally entitled to be in and remain in Canada and make your permanent home in Alberta and to be physically present in Alberta for at least 183 days in any 12 month period
When moving from another province there may be a waiting period before you become eligible for coverage under AHCIP

- Alberta Residency
- Alberta Drivers License, or bill such as utilities or phone bill
- Identity
- Government issued Drivers License, Passport or Birth Certificate
- Legal entitlement to be in and remain in Canada
- Canadian Passport, Canadian Birth Certificate, Canada entry documents

Step 4: Submit the application
In person at our Registry – Processing time can take up to 5 days
